• Please complete the form, noting any special guidance at the bottom. A LOGIX rep will contact you, or your customer, according to your instructions.
• Once LOGIX receives your referral form, you will know within 3 business days if your customer referral is accepted and eligible for the partner program.
• If the lead becomes a LOGIX customer within 120 days, you will receive credit per your partner agreement.
• Please try to pre-qualify your contacts for LOGIX services, when possible. Prospects should have a general understanding of LOGIX phone, Internet or data center offerings.
